About Us
Advanced Care Partners was established to improve the healthcare delivery system and close the loop in communication between providers. Based on the “Triple Aim Approach” of high quality, lower costs and improving the patient experience through population health, Advanced Care Partners will help primary care providers truly become the gate keeper and advocate for their patients. Following Advanced Care Partners formation in the first quarter of 2019, we began recruiting physicians and physician practices through-out the tri-county area of south Florida (Palm Beach, Broward and Miami-Dade).
Advanced Care Partners deliberately reached out to like-minded and goal-oriented providers with experience in value-based care within other fields of the health care industry, including Medicare Advantage, who wanted to apply these same principles to their entire practice.
Advanced Care Partner’s mission is the improve the relationship between the patient and the primary care provider in order to properly ensure that every individual’s healthcare needs are addressed on a more personalized level to help coordinate services and execute the patient’s customized plan. Our collective interdisciplinary team of as well as our partnerships with local Universities demonstrate Advanced Care Partner’s dedication to the patient’s health and well-being to deliver quality care, improved outcomes, appropriate utilization and exceptional engagement.
